The first thing you should do is to download the Pag-IBIG Loyalty Card Plus application form (HQP-PFF-108 V07). After downloading the form, open the document using a free PDF reader such as Adobe Acrobat. If you are using Chrome on desktop, the form will automatically open in a new tab so you don’t need to use a PDF reader.
Print the LCP application form by clicking File > Print or by pressing Ctrl + P on your keyboard. Make sure that you are connected to your printer when doing so.
For best results, print the LCP application form on long bond paper (8.5″ x 13″). However, you can print it on A4 or any other paper size as long as you set the page sizing to fit.
